Referral pathways

Referrals can be made for patients within the following Primary Care Trusts:

  • Cheshire and Merseyside
  • Cumbria and Lancashire
  • Thames Valley
  • Kent and Medway

By:

  • General practitioners
  • Community optometrists
To make a referral, please contact your nearest Patient Referral Centre. The Centre will then contact the patient to arrange the first outpatient appointment.
